1) I give permission for my dues to be used In the pursuit of the Vision of the OTOC Mutual Benefit Association (OTOC MBA) of which I desire to seek Membership. 

 

2) I understand the annual dues are $60; however, $5 /mo will be sent in by me to honor my commitment to be active and pro-active in the 12-month-long Health, Education and Wealth program.
