1) I give permission for my dues to be used in the pursuit of the Vision of the SHoFI Mutual Benefit Association of which I desire to seek Membership.
2) I understand the annual dues are $60; however, $5 /mo will be sent in --WHEN THE START DATE IS GIVEN-- by me to honor my commitment to be active and pro-active in the 12-month-long Health, Education and Wealth program (OTOC, a mutual benefit association) designed to improve my health by 150% and other great education and wealth improvements.
